Dr. Ichito Kuriharai, portrayed by Sho Sakurai, is employed at a medical clinic in Nagano Prefecture. His schedule is perpetually packed due to a shortage of medical staff and a steady stream of patients. This often forces him to treat individuals outside his area of expertise and endure long stretches without rest. Ichito aspires to join a larger hospital, hoping it will afford him more personal time to be with his wife, Haruna, played by Aoi Miyazaki, and allow him to focus on his specialty. Despite this, he hesitates to leave the clinic, as it would exacerbate the doctor shortage and sever his bonds with the patients. One day, while sorting through the possessions of a terminal cancer patient who has passed away, Ichito finds a letter addressed to him. The letter expresses gratitude for the care he provided, despite being unable to cure the illness. This moment leads Ichito to a profound realization about the medical chart he holds, which is filled with his personal notes and reflections.
